When time is of the essence, providing quality of life is essential. Hospice care supports individuals with life-limiting illnesses at home or in care facilities, and the compassionate team at Hillcrest Hospice is devoted to making every minute count. Our trained professionals strive to enable your loved one to live life to the fullest potential while providing you and other family members with counseling and support through the entire hospice journey.

Compassionate Care Team

Medical Directors oversee an interdisciplinary team comprised of registered nurses, social workers, chaplains, aides, grief counselors and others. The team uses a holistic approach, not just caring for the physical needs of the individual, but also the emotional, social, spiritual and practical needs. A customized care plan is created for each patient and family, focusing on dignity and comfort rather than curative treatments, ensuring quality of life for your loved one. The care team at Hillcrest Hospice is on call 24 hours a day, seven days a week, to make sure your loved one will always be comfortable. Our team members consistently receive excellent reviews from families telling us how the experience enhanced their lives.

Supportive Programs for Patients & Families

Bereavement (grief) counseling and spiritual support are available to families for up to 13 months after their loved one passes. Our “No One Dies Alone” program ensures that everyone entrusted to our care has the support they need at their most critical time. We can assist with creation of a “CaringBridge” website, allowing families to communicate information and status updates with each other, as well as send special messages to their loved one. In addition, our Community Support Groups can help families and friends cope with the loss of a loved one. Continuous Care

According to Medicare data, Hillcrest Hospice is the only hospice providing Continuous Care for hospice patients in Nebraska. It is one of four levels of hospice care covered by the Medicare and Medicaid Hospice Benefits. This 24-hour, around-the-clock nursing level of care is provided in the home for brief periods of time when the patient is experiencing any symptom. It is designed to honor the person’s wish to remain at home by providing the care needed to control their symptoms and alleviate the crisis. Home is defined by the person, and continuous care can be provided at home, an assisted living or a nursing home – wherever the patient calls home.

In addition, if the patient reaches their final seven days, Hillcrest Hospice can also dedicate a Registered Nurse to be present with the patient for up to four hours a day. This RN care does not require a symptom and is simply placed in order to provide the highest intensity of end-of-life care possible – to fulfill the Hillcrest mission. For more info, call (402) 934-2282 or click the contact us button below.
