Nestled within Hillcrest Country Estates’ beautifully landscaped 44-acre continuing care retirement community (CCRC), the Rehab Cottage offers a unique setting with only 22 private suites and the first “cottage-style”, exclusively post-acute rehabilitation center in the area with a specialized focus on geriatric rehabilitation and recovery.

After rating more than 15,000 skilled nursing facilities nationally, U.S. News & World Report named Hillcrest Country Estates Cottages as one of the “Best Nursing Homes for 2019-2020.”

On your road to recovery — whether you’re recuperating from an injury, illness, surgery or hospital stay — you’ll feel an incredible sense of warmth, compassion and support from a team that is 100% dedicated to your recovery. Our exceptional team will design an individualized post-acute care plan to ensure a successful transition back to your normal lifestyle.

Cottage Amenitites

While we meet most any rehabilitative need, our most common cases include:

  • Post-operative recovery
  • Joint replacement or fractures
  • Wound care
  • Stroke recovery
  • Cardiac rehabilitation
  • General debilitation or weakness
  • Recovery from illnesses like pneumonia

The Universal Worker Approach

The care teams have adopted a universal worker approach, which means the same team members complete almost all of the tasks in their assigned cottage, giving them more time to build solid family-like relationships with the Elders. A Cottage Guide oversees the daily operations of two cottages, and assists with activities, outings, family concerns and social work issues. The Elder Assistants, with support from licensed and registered nurses, provide direct care, while also handling cooking, cleaning and social activities – and participating regularly with family members in Quality Circles.
