Following surgery, illness or injury, Hillcrest offers post-acute care and short-term rehabilitation services to help you recover and return to your previous level of activity. Let us help you get back to the lifestyle you’re accustomed to with our continuum of care, from in-patient rehab in our post-acute rehabilitation centers to skilled home health care in the comfort of your own home, with follow-up care offered through our outpatient physical therapy clinic.

Short-term, in-patient rehabilitation is the most intense level of post-acute rehab offered following release from a hospital stay, with patients receiving skilled nursing, wound care, and physical, occupational and speech therapy, if needed, for up to seven days a week. The average length of stay is between 14-22 days.

Following discharge from our in-patient rehabilitation centers, most individuals return home or to their independent or assisted living community. For those who return home, skilled home health services are provided following the transition. From skilled nursing, wound care, and telehealth vital sign monitoring, to in-home physical, occupational and speech therapy, home health services assist homebound patients with their recovery.

Another advantage of Hillcrest’s continuum is the shared electronic health record (EHR), allowing team members to access your history and medical charts to ensure continuity of care. No other healthcare provider in Omaha has this level of technology available.

The final step in most patients’ post-acute care plan is outpatient therapy. A physician will prescribe physical, occupational or speech therapy in an outpatient clinic setting to follow either in-patient rehab or home health services. Therapists work with patients to regain strength, balance, motion and the ability to perform the activities of daily living. Therapy may last from a few weeks to a few months depending on the severity of the patient’s condition, as well as how quickly they respond to therapy.

Once recovered, individuals should consider joining a wellness program to maintain their health, wellness and independence. By improving strength, balance and cardiopulmonary fitness, aging adults can live a longer, healthier life.
